Having a Child Evaluated for Assistive Technology
by Your Local UAAACT Team

Before the evaluation can take place the UAAACT Team must receive a referral form. Other information may be required before the evaluation, the UAAACT Team will contact those persons concerned for this information.
  1. Initial Contact- A case manager from your local UAAACT Team is assigned, who is in charge of contacting and scheduling with parents, the principal, teachers and any other person who may contribute to a child's assistive technology needs.
  2. Two to three days prior to the scheduled assessment, the case manager confirms the date with the student's teacher and parents.
  3. The assessment is conducted (not necessarily in this order and including all aspects listed)
    1. The UAAACT Team will review the Student's IEP/504, and any recent testing.
    2. Conduct student observation.
    3. Interview parents, teachers and other appropriate members of the student's educational team.
    4. Have the student try assistive technology products.
  4. Post Assessment Meeting with IEP Team and Parents
    1. The UAAACT Team will share information and explore resources and plans for implementation of assistive technology devices.
    2. Provide one or more assistive technology devices for a trial use period of 30 days to three months as necessary.
    3. Provide basic instruction on how to use the assistive technology product or products the student will use during the trial use period.
    4. Schedule a follow-up visit usually within four weeks, to provide additional training, add vocabulary to an augmentative communication device and/or review student progress.
    5. Leave IEP team members with data collection forms and provide instructions for their use (Collecting accurate data is very important in determining if a student can benefit from the use of the selected assistive technology product).
    6. Provide the students educational team including the student's parents with the case manager's contact information.
  5. Reporting
    1. A formal report will be completed within approximately two weeks of when the assessment is complete.
      1. The Assessment may take several sessions over two or three months.
    2. The formal report should be sent to the student's parents, a member or the student's IEP team and a copy should be placed in the student's file.
    3. If follow-up visits are conducted after the formal report has been completed, follow-up status notes may be provided to the IEP team and kept in team files.
  6. Follow-up
    1. If needed the case manager works with the team leader to schedule additional visits with the student, IEP team members, and parents.
